Obtain A Repeat Prescription?

If you are receiving regular medication, you will be given a repeat slip.

The order instructions will be given on the bottom of this slip. Either send it through the post, bring it into the surgery and place it in the red box in reception or complete the online form below. Please give 48 hours' notice.

We do not accept repeat prescription requests over the telephone.
